About the service

Specialist Palliative care is the active holistic care of patients with advanced, progressive life limiting illness. This includes the management of complex symptoms and the provision of psychological, social, and spiritual support in line with best practice to improve quality of life.

Specialist palliative care services manage complex palliative care problems that cannot be dealt with by core services alone. Anyone over the age of 18 yrs of age with a palliative diagnosis with complex needs who is registered with a GP in West Cheshire can ask to be referred into the service.

We believe in offering fair and impartial specialist palliative care which is not dependant on gender, disease process, ethnicity, cultural or religious backgrounds.

The team aims to provide the following:

  • Advice on the management of pain and other symptoms that may be of the most concern
  • An opportunity to discuss physical, emotional, and social needs.
  • Offer advice and support to families and carers.
  • Advice and support to make choices, planning for future care, advanced care planning and preferred place of care discussion.
  • Provide direction to other appropriate health and social care services required such as bereavement support, complimentary therapies, living well services, financial advice.
  • Therapy support to optimise independence and the quality of life for patients with a complex palliative care diagnosis.
  • To maximise dignity and reduce the extent to which disease impacts an individual’s physical and psychological functioning, including advice/support/education regarding appropriate treatment, intervention, and equipment.

Who is in the team?

The Community Specialist Palliative Care Team (CSPCT) provides a multi-professional approach. Roles within the team include - Clinical Nurse Specialists (CNS) Occupational Therapist Physiotherapists Therapy assistant.

We are also supported by Palliative Medical Consultants and work closely with our local Hospice and Care Community and Specialist Teams

Monday to Friday 8.30am – 5.00pm (excluding bank holidays)
