Here at CAMHS, we want to make sure you get the best help possible.
In order to help us do this, we want to get to know you by talking to you and those who support you.
With your permission we would like to discuss some things we talk about together, at team meetings and with other services, to make sure you get the most out of your time here.
Sometimes notes are taken, which you can ask to see at any time, so that we can keep track and save you from repeating yourself. In Cheshire, these notes can be read by accessing a system called the Cheshire Care Record, for which we must ask your permission to view.
Your information will only be viewed by people directly involved in your support, such as to your GP and your CAMHS case worker.
This will really help us to have a wider network of support for you. If you wish to not go ahead with this, please tell your GP and they will make sure it does not. Please also feel free to ask us at CAMHS if you have any questions about this.
